Master’s Level Mental Health Case Manager – Adult Outpatient (Grants Pass)


Case Manager provides recovery oriented services to individuals with mental illness and/or co-occurring disorders. Facilitates and coordinates access to needed services and supports within the community. Completes assessments and treatment plans. Implements treatment plan for clients on caseload.


The Master’s level Case Manager completes screenings, assessments and treatment plans and carries out individual treatment plans for clients on his/her caseload, which may include providing assistance with accessing entitlements, housing, accessing health care, etc. Drives agency vehicle and transports clients as required. The majority of services are delivered in the community. Submits timely documentation of services provided using Electronic Health Records and collaborative documentation. Manages time and caseload effectively.


Candidate must possess a Master's degree in behavioral science or a related field, an understanding of co-occurring disorders, and related experience. Candidate must be able to work independently and flexibly, under general supervision. Experience with substance abuse treatment for dually diagnosed clients preferred. Computer proficiency required to use Electronic Health Records. Valid OR driver's license and excellent driving record required. Must successfully complete state-required background and DMV checks.
