Description

The Manager, Clinical Service Delivery - Nursing Home Services will:

Accountable for the day to day operation and fiscal management of the clinical delivery of services provided to include Skilled Nursing Facility (SNF) rounding, Long Term Care (LTC) rounding, Medical House Calls (MHC) and Dialysis Rounding.

In partnership with the NHS leadership team sets standards of clinical quality, productivity, safety, and service. Ensures SNF facility performance standards related to provider specific processes to ensure overall safety, cost, quality and service for patients receiving SNF care and LTC care.

Develops service delivery plans, business strategies, care processes and system design to support safety, service, quality and productivity and cost.

Provide leadership and direction to Nursing Home Services ARNPs and the multi-disciplinary professional practice team providing care for our members during sub-acute or residential stays at community nursing homes. Develops coordinated care and service planning and evaluation.

Provide budget management, human resource management and assurance of compliance with regulatory and practice standards.

Provide high quality, consumer-centered care and services by partnering and collaborating with other Nursing Home Services and Continuing Care managers, leadership and medical staff organizational-wide, clinic staff and multiple community skilled nursing facility providers.

Qualifications

Education

Minimum: Bachelor’s Degree in Nursing

Preferred: Master’s Degree in Nursing or Geriatric Nurse Practitioner

Knowledge/Skills

Minimum: Advanced knowledge of healthcare systems, and department or division specific work processes, standards, regulations and systems. Ability to work independently and multi-task. Organizational, communication, critical thinking, decision making, interpersonal, problem solving, analytical, team building and change management skills.

Experience

Minimum: 3-years’ experience in health services management & successful work with teams including physicians at a middle management level and prior experience in program development & manager/agent of change.

Preferred: 5-years’ experience in managing care management teams. Experience in care management principles to include utilization management and discharge planning.

License/Certifications

Minimum:Current WA State RN license, Washington State Driver’s license

Preferred License/Certifications: ARNP license and certification
